​​How Katie Malley Therapy uses your personal data:

​Your personal data is confidential and will not be shared with any third parties.  Personal details are either taken and kept for the purposes of keeping you safe during sessions, planning therapy sessions and enhancing your experience or for the purpose of booking you on to a course, keeping you safe during classes/courses and contacting you regarding the course. 

We are required to keep records for a period of time for insurance purposes.  Your details are kept securely and, if no longer required by Katie Malley Therapy, destroyed by shredding after five years for adults. For clients under the age of 18 records will be kept up until their 18th Birthday and then for a further five years after their 18th Birthday.

This privacy notice provides you with details of how we collect and process your personal data including any information you may provide through our site when you book a service, sign up to any future newsletters or complete a course booking form.

Katie Malley Therapy is responsible for your personal data (referred to as “we”, “us” or “our” in this privacy notice).

Course Bookings

​Identity Data may include your first name, last name and that of your child/children.

​​Contact Data may include your home address, email address and telephone numbers.

​Financial Data is not collected. All payments are collected either by Booking That In (booking service) BACS, cash, cheque or gift voucher.

Transaction Data may include details about payments made by yourself for course bookings or appointments.

Your personal data will be stored and be accessible by Katie Malley Therapy.

Your information will not be shared with any third party companies.

​We do not store credit card details and we never share customer details with any third parties

​We will only contact you with regards to Katie Malley Therapy services and associated matters and will never send spam to you.

​​Client Consultation Forms

Identity Data may include your first name, last name, marital status, title, date of birth and gender.  Identity Data may also include details of your child/children and family makeup

Contact Data may include your home address, email address and telephone numbers and that of your next of kin

Health Data: The medical and emotional questions are required so that we can provide a holistic treatment for you, indentify the presenting issues and what you wish to get from the therapy

Sensitive Data: We do collect some Sensitive Data about you. Sensitive data refers to data that includes details about your medical background, race or ethnicity, religious or philosophical beliefs, sex life, sexual orientation. We do not collect any information about criminal convictions and offences, unless this is shared during therapy sessions

Financial Data is not collected. All payments are collected either by BACS, cash, cheque or gift voucher.

Transaction Data may include details about payments made by yourself for treatments.

Your personal data will be stored and be accessible by Katie Malley Therapy.

Your information will not be shared with any third party companies.

We will only contact you with regards to Katie Malley Therapy services and associated matters and will never send spam to you.

​​Data Retention:

By law we have to keep basic information about our customers (including Contact, Identity, Financial and Transaction Data) for six years after they cease being customers for insurance and tax purposes.

​Medical Malpractice requests that we keep consultation files on record for seven years once you have received your last treatment. After this date they will be shredded.

​We will only retain your personal data for as long as necessary to fulfil the purposes we collected it for, including for the purposes of satisfying any legal, accounting, or medical requirements.

​Disclosing Information

We will not sell, distribute or lease your personal information to third parties unless we have your permission or are required by law to do so. We may use your personal information to send you promotional information about third parties which we think you may find interesting if you tell us that you wish this to happen.

 

Webserver Logs

For statistical purposes, we may gather certain information automatically and store it in log files. These log files contain standard information collected by web servers, such as client IP addresses, browser type, internet service provider (ISP), operating system, etc. This information will only be used internally and is not distributed or sold to a third party. We may use this information, which does not identify individual users, to analyse trends, to administer the website and to gather demographic information about our user base as a whole.

Booking Terms & Conditions

​

The full amount is payable at the time of booking. Your place/ service is secure on full payment. All contracts and agreements and/ or booking forms (where necessary) must be completed before commencing any sessions or services, not including the initial consultation.

​

Cancellations

24 hours’ notice is required to cancel your therapy session without incurring a charge. If your appointment is on a Monday, please cancel on Friday. If less than 24 hours’ notice is provided, you will be charged the full cost of your session.

 

If a health or insurance company is paying for your treatment, they may make you liable for the charge. Your treatment sessions could be suspended dependant on their policy.

​

The same policy applies for rearranging appointments. 

 

Non-attendance & Lateness

If you fail to attend a session without any notice you will be charged the full amount, payment will need to be received before booking any further sessions, and your file will be placed on hold. If you are more than 15 minutes late to your session, the session will not go ahead as there is not enough time remaining and sessions can not be extended. You will be charged the full price for this session.

​​Cancellation of Courses & Workshops - NOT THERAPY SESSIONS
Cancellation within 2 weeks of the course start date or withdrawing during a course is classed as a cancellation and no refund for the course or remaining classes will be given. Cancellation of the course outside of 2 weeks will receive a full refund minus £5.00 administration fees. Katie Malley Therapy reserves the right to cancel/suspend a course/workshop if there is an issue with the venue, class numbers, or unforeseen circumstances. In the event of a cancellation made by Katie Malley Therapy a full refund will be given. Our liability for any such cancellation shall be limited to the payment credit made and we shall not be liable for any losses of any kind incurred by you as a result of such cancellation. The credit will be made within 14 days of the date on which we agree that you are to be credited to your payment method. We may offer an alternative course or venue.

 

Missed Classes

We understand that on occasion a class maybe missed. We are under no obligation to refund for missed classes. Our costs remain the same regardless of attendance.

​

Absence Due to Illness

Please do not attend classes if you are showing signs of upset stomach with diarrhoea or sickness, contagious disease any other member of your family is showing signs of sickness.

​Refunds/discounts are not given for classes missed due to illness. 

 

​Safety

This is always of paramount concern. However, we are not liable for any loss, damage or injury that may occur whilst attending our classes, courses or workshops.

​You are bound to disclose any information regarding your or your child's health relevant to the practice of the class you are attending.

​You are responsible for all applications of skills you may practice outside the classes both now & in the future.
